Driver Fit for Duty form and Site induction information
- By completing and submitting this form you agree and fully comply with the following: 1. I hold a current and valid licence to drive this vehicle combination. 2. I have had or will have my legal rest break(s) and in my opinion am fit to perform my driving. 3. I have completed a pre-check and will not exceed legal axle weight or GVM/GCM for the vehicle combination I am driving. 4. I am not fatigued; do not have blood alcohol over 00; no illegal drugs in my system and have not taken prescription meds that will cause drowsiness or affect my safe driving ability. 5. I have not been advised by my doctor that I am unfit for work. 6. My mental health will not impede my ability to drive. Site Induction requirements and information:
- 1. Hi-Vis top and safety boots must be worn onsite
- 2. Do not use engine break whilst arriving at or driving around our site
- 3. Announce arrival and departure using UHF 36
- 4. Give entering vehicles access before exiting the site
- 5. Obey all speed limits onsite and ensure loader is given right of way
- 6. Emergency meeting point is at the front entrance gate 7. First aid kit and toilet located in office
