Driver Fit for Duty form and Site induction information
By completing and submitting this form you agree and fully comply with the following:
- I hold a current and valid licence to drive this vehicle combination.
- I have had or will have my legal rest break(s) and in my opinion am fit to perform my driving.
- I have completed a pre-check and will not exceed legal axle weight or GVM/GCM for the vehicle combination I am driving.
- I am not fatigued; do not have blood alcohol over 00; no illegal drugs in my system and have not taken prescription meds that will cause drowsiness or affect my safe driving ability.
- I have not been advised by my doctor that I am unfit for work.
- My mental health will not impede my ability to drive.
Site Induction requirements and information:
- Hi-Vis top and safety boots must be worn onsite
- Do not use engine break whilst arriving at or driving around our site
- Announce arrival and departure using UHF 36
- Give entering vehicles access before exiting the site
- Obey all speed limits onsite and ensure loader is given right of way
- Emergency meeting point is at the front entrance gate
- First aid kit and toilet located in office
